Identifying barriers and recommending modifications for safety and accessibility.
Powered mobility devices, walkers, wheelchairs, hospital beds, pressure mattresses, shower and toileting equipment.
Assisting with access improvements to toilets, bathrooms and other problem access points to promote safe living at home.
Assessing risks and implementing interventions to reduce falls, with education and suitable aid recommendations.
Cognitive screens to identify deficits, with support developing routines and strategies for memory and concentration.
Detailed reports and letters of recommendation for housing, equipment requests, and NDIS access and plan reviews.
We are currently accepting referrals for older adults across Support at Home (SAH), NDIS and DVA funding models.
Your Support at Home case manager can refer to us directly to provide assistance with your home care needs.
